Freddie Dwozan, RPh
Founder and Owner
Freddie began his career in one of the most trusted professions, pharmacy services. As a small business owner he has dedicated his life to help Georgians who are sick and in need of care and comfort. Like all of his employees, he is Georgia resident and lives and works in the organization’s service area. He is a University of Georgia Pharmacy school graduate and a practicing registered pharmacist. He is a member of the Georgia Pharmaceutical Association and serves on the Board of Directors of the Dodge County Hospital Foundation.
Freddie strives to employee only the best, brightest, and most dedicated professionals in their respected fields who share his ideal of service above self.

Marci Dwozan McCranie, RN, BSN
Director of Human Resources and Customer Service
Marci continued her family’s commitment to serve others and attained a BS in Nursing from Georgia College and State University. She is dedicated first and foremost to our patients and families as Customer Service Director. She also supports our most important asset, our people, in her Human Resource role.
Tammy Peterson
CFO
Tammy has provided assistance to patients with insurance benefit coordination since the humble beginnings of company in 1997, and has over twenty years of tenure with the parent organization. Her more than a decade of service has focused on helping families understand their insurance benefits to maximize their access to the services they are entitled to receive. Tammy holds a BS from Mercer University.
Paul Elmore, CHA
COO and Executive Director
Paul has served others in the health care industry since 1985 and has led the operations of Hospice Care Options since 2000; dedicated to upholding the organization’s founding mission of service to those in need. He earned a BBA from the University of Georgia and is a Certified Hospice Administrator. He is a member of the National Council of Hospice and Palliative Professionals and serves on the Board of Directors of the Georgia Hospice and Palliative Care Organization.
Professional Leadership
Diane Rowland, RN
Director of Patient Care Services
Diane has over a decade of experience in nursing care in numerous settings including critical care and palliative nursing. A Macon State College graduate, she has dedicated her career to help and bring comfort to critically ill patients and families. With her extensive clinical expertise, she provides guidance to the organization's staff in the provision of nursing services. Diane is a member of the Hospice & Palliative Care Nurses Association.
Tina Cansler Clark, M. DIV
Director of Regional Services
Tina has spent 30 years of her professional career in caregiving work. She’s been a member of the Hospice Care Options team for 10 years, working first as a chaplain and transitioning into the role of Regional Services Director. Her areas of oversight include social services, spiritual care and volunteer services as well as daily operations. Tina earned her undergraduate and M.Ed. from West Georgia College and her M.Div from Mercer University’s McAfee School of Theology. She feels that working in end of life care keeps her “close to the flame” and she is grateful for the opportunity to enter the lives of our patients and their families.
